Description

This short programme provides nurses and midwives with the most up-to-date information regarding policy and standards. It will enhance their understanding of nursing care plans, reflecting on the past, present and future use of care planning and its importance in the workplace.  It will focus on the need for comprehensive assessment, including risk assessment and care planning.  Participants will be provided with practical tips on how to prepare for and carry out a comprehensive assessment, enabling them to develop a person-centred care plan.
Learning outcomes:  
 
At the end of this virtual classroom programme participants will be able to:
-   Be competent in the assessment, planning, implementing and evaluation process as outlined by NMBI.
-   Be knowledgeable on risk assessment and management process relating to the Patient/Person.
-   Have a clear understanding on the relevance of ‘road mapping’ and linkage throughout the care plan and Person-Centred Care Support Plans.
-   Be knowledgeable on  assessment and care plan domains and best practice relating to assessment of the patient/resident/service user
-   Develop SMARTER goals with the  patient/resident/service user and understand the time frame of each assessment and commencement of a timely care plan as per national standards
-   Provide clear formulas on  care planning process in collaboration with the person from initial assessment, Need identification, Goals and interventions.
-   Develop a comprehensive  care plan as a group using clear structure of Assessment, need identification, goals, interventions and evaluation.
 
Note:  On completion of this course a certificate of attendance in a digital form and will be sent by email to each participant.  Please note places are limited and must be booked in advance; you need a reliable computer and internet access and please ensure a correct email is provided when registering.
