Description

This short introductory online course provides nurses/midwives who work in the primary healthcare setting with knowledge and skills to develop and apply the principles of self-management of chronic illnesses.  In this programme you will discover the most common chronic diseases  and learn how to assess clients with ongoing illness and to develop, implement and evaluate planned care and self-management strategies.  This is an ideal professional development programme to gain essential skills to better support these patients and provide you with the knowledge and skills in doing so.  
 
Learning outcomes:  
 
-  Identify how Chronic Disease Management (CDM) impacts on the provision of care in primary healthcare setting.
-  Discuss the affect of CDM on patient morbidity and mortality.
-  Discuss the research both local and international which describes an evidence-based approach to the delivery of service.   
-  Have the skills to perform a detailed audit to identify current cohorts.
-  Set up CDM Clinics using current guidelines and policies.
-  Deliver care to patients with CDM using an evidenced based approach.
-  Have an understanding of the multidisciplinary approach to the care of a patient with chronic disease.
-  Identify the role of the primary, secondary and tertiary care in the provision of services to patients with Chronic Disease.
-  Evaluate the care provided especially in the prevention of secondary diseases and the improvement of patient quality of life issues.
